Description
We are working with a healthcare provider who has been awarded a contract to deliver dermatology assessment, diagnosis and treatment within dedicated locality hubs in Eastern & Coastal Kent. Each locality will be supported by an integrated team consisting of Consultants, Nurse Specialists, Nursing Support and Admin Support.

The service will be available to patients between 8.30am - 5.30pm Monday to Friday. Extended hours of operation, to include Saturdays and evening provision are to be considered.

Primary Objectives
•To deliver locality based dermatology consultant led service
•To achieve the best health care outcomes for patients with dermatology conditions
•To deliver one stop assessment, diagnosis and treatment service for all new patients

Secondary Objectives
•To support GP education to raise awareness of dermatology conditions through development of training plan and web based information
•To facilitate ability of GP's to work within dermatology service as means of increasing skills and knowledge base
•To provide assessment, agree realistic goals with patients, treat appropriately using an evidenced based approach and evaluate outcomes

General
This service will provide assessment, diagnosis and treatment of all dermatology patients with the following exceptions: Rapid Access referral, diagnosis and treatment; Patients who require PUVA treatment; Suspected connective tissue disorders; Systemic illness with a rash; Cutaneous vasculitis. These patients will be referred in the first instance to secondary care.
